The Patient Safety and Risk Officer will primarily be responsible for:

  • Managing the information within the Datix database for incidents and risk.
  • Analysing and presenting information in various formats for groups across the trust and externally.
  • Supporting the Quality and Standards Division in promoting good risk management practice throughout the Trust.
This is an exciting time to join the Trust, as we make progress towards implementing the Patient Safety Incident Response Framework which will help us to learn more effectively from incidents.

The successful candidate will be enthusiastic, flexible, willing to learn and able to use their own initiative to organise and prioritise their workload. The candidate must have a commitment to quality improvement with excellent communication and interpersonal skills and attention to detail. They should have a background in working with teams from different professional backgrounds.

Experience of working with Datix risk management software is an advantage, but not essential, as full training will be provided.

The Christie is one of Europe’s leading cancer centres, treating over 60,000 patients a year. Incident reporting and risk management

1. Ensure that all information within incidents and risk modules of Datix is accurate and up-to-date for incident investigation and reporting purposes.

1. Use initiative to ensure that all required information has been entered onto the incident report and highlight any omissions.

1. Allocate handlers for Grade 1 and 2 incidents as soon as the report is received, to ensure appropriate investigation.

1. Grant and manage access to the Datix databases and ensure relevant people receive appropriate notifications for information (e.g. moving and handling, infection control, security, finance, safeguarding) and forward incident reports to the appropriate Senior Manager for investigation.

1. Highlight where any reports to external organisations need to be made, e.g. HSE, MHRA, and be responsible for submitting the report and providing additional information as required.

1. Highlight any failures to complete investigations within the designated timescale to handler, and initiate escalation process to Patient Safety and Risk Manager and Patient Safety and Risk Lead in a staged process.

1. Update the database with any new or additional information relating to an incident report during or following investigation.

1. Follow process for closing off incidents and attach any information relating to the incident that subsequently becomes available. Ensure electronic filing system is well maintained.

1. Take responsibility for updating the reports schedule in line with the requirements of the divisions and various trust committees.

1. Plan, design and produce scheduled and ad hoc reports from Datix and Datixweb for managers and committees, to include trend analysis and recommendations for change where appropriate.

1. Collate and present statistical information on incidents for the divisional and departmental performance and board reports.

1. Collate and produce papers for weekly Executive Review Group and Friday FoCUS and provide administrative support to these meetings.

1. Undertake the process, including escalation, for the distribution of safety alerts and notices issued through the Central Alerting System and chase outstanding responses to ensure full compliance with required actions within the set deadlines.

1. Act as liaison point for other organisations for incidents that are their responsibility or are being reported by another organisation about The Christie services.

1. Act as secretary to the Patient Safety Committee and Risk & Quality Governance Committee. Provide any administrative support that may be required by the Committees. Collate and send out agendas and papers.

1. Contribute to the quarterly patient safety report for incidents and safety alerts. This will include the presentation of graphical and written information and involves the analysis of several fields of information.

1. Support the roll-out of enhancements and upgrades to Datixweb, providing assistance and training.

1. Support and participate in audit processes to assess compliance with national guidance and standards for risk and patient safety.

1. Provide systems administrator support for Datix to users: this includes access, security and some training and requires an excellent working knowledge of all the modules in Datix.

1. Support month end quality checks for patient safety incidents before they are uploaded to the National Reporting and Learning System (NRLS).

1. Attend departmental meetings to provide support for the local management of incidents and the progress of investigations.

General

1. The post holder will need to be able to prioritise their workload and work with some independence to ensure the needs of the service are met and that all standards are complied with. The post holder will be expected to work to clearly defined procedures.

1. Provide support as required by senior members of the Quality and Standards team in the absence of the Patient Safety and Risk Manager and the Patient Safety and Risk Lead. Support the wider Quality & Standards team where appropriate, particularly with use of the Datix database.

1. Promote good risk management and the philosophy that every untoward event is an opportunity for learning by preparing posters, setting up displays and attending departmental meetings to advise on best practice etc

RISK MANAGEMENT

It is a standard element of the role and responsibility of all staff of the Trust that they fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards.

RECORDS MANAGEMENT/DATA PROTECTION ACT

As an employee of the Trust, you have a legal responsibility for all records (including patient health, financial, personal and administrative) that you gather or use as part of your work within the trust. The records may be paper, electronic, microfiche, audio or videotapes, x-ray images. You must consult your manager if you have any doubt as to the correct management of the records with which you work.

HEALTH AND SAFETY REQUIREMENTS

Employees are required to co-operate with management to enable the Trust to meet its own legal duties and to report any circumstances that may compromise the health, safety and welfare of those affected by the Trust undertakings.

As a Trust employee you are required to uphold the confidentiality of all records held by the trust, whether patient records or trust information. This duty lasts indefinitely and will continue after you leave the trust employment.

All Information which identifies individuals in whatever form (paper/pictures, electronic data/images or voice) is covered by the 1998 Data Protection Act and should be managed in accordance with this legislation.
